PLEASE NOTE: ONCE THIS FORM IS SUBMITTED, IT CAN TAKE UP TO 5-7 DAYS TO SET UP YOUR ACCOUNT AND SUPPLY YOU WITH USERNAMES/PASSWORDS FOR YOUR STUDENTS.

Submit this form to create accounts for your students for the Flourishing Life Assessment. Anonymous usernames and passwords will be generated that correspond with your alphabetical class list.

PLEASE READ THE INSTRUCTIONS CAREFULLY
Use one form for each program facilitator in your school. Please provide your school/organization, the facilitator’s first and last name, the number of classes you will be facilitating, the class ID for each class, and the number of students in that class.

Please provide classroom ID’s and number of students for each class you want set up with the Flourishing Life Assessment.
